[1] Iopodate has been used successfully in treating acute thyroid hormone poisoning. [2] We have observed that oral administration of ipodate preoperatively for two to three weeks in patients with refractory Graves' disease results in a rapid and effective conversion of hyperthyroid state to euthyroid state without any side effects. Patients and Methods Since 1997 seven patients with Graves' disease and with refractory hyperthyroidism have undergone subtotal thyroidectomy after having been prepared with oral ipodate. Six patients were operated on in the State of Massachusetts, USA, and one patient was operated in Tehran, Iran. There were 5 females and two males patients. Their ages ranged from 20 to 52 years. Their hyperthyroid state could not be controlled despite intensive efforts by endocrinologists; therefore they were referred for thyroidectomy. TSH was below 0.02 micro U ml in all patients. T4 on average was 20% to 30% above the upper normal limit, and T3 was 30 to 50 percent above the upper normal range. Two patients were not compliant with their prescribed medications. One of the these patients was a 20 year-old cocaine addict female with Graves' disease, who because of a combination of hyperthyroidism and cocaine intake, had a very unstable condition, both mentally and physically. One patient with Graves' disease had developed unstable hyperthyroidism because of an inter current infection, and a patient with a very large goiter had previously been treated twice with radioactive iodine without adequate response. All seven patients had large goiters. Preoperatively patients were continued on Tapazol or PTU, and given dexamethasone 1 mg daily and propranolol 10 to 20 mg TID, in addition to ipodate. Patients drank 1 ml to 1.5 ml of an intravenous CT contrast agent containing ipodate or iohexal twice daily. This volume contained about 500 mg to 800 mg and didanosine and dexamethasone.
