While it is acknowledged that some progress has been achieved with improving laws and regulations, most notably with regard to parallel importation, PhRMA member companies continue to face a number of serious barriers to conducting business in Vietnam in several main areas, including those related to Intellectual Property Protection. More specifically, this refers to compulsory licensing, infringement of registered trademarks, inadequate enforcement, insufficient protection for product trade dress, and importantly, counterfeiting - including that of medicines. These fundamental problems need to be addressed. Thus, PhRMA requests that Vietnam be included in the 2003 "Special 301" Watch List. Intellectual Property Protection Compulsory Licensing Under existing law, the National Office of Industrial Property NOIP ; may order a compulsory license. PhRMA believes that patent compulsory licensing systems are seriously counter-productive except in cases of national emergency. Consequently, PhRMA maintains that the current law should be amended to eliminate the existing grounds for granting non-voluntary licenses, and to include conditions provided in the U.S.-Vietnam Bilateral Trade Agreement BTA ; and the World Trade Organization WTO ; Trade Related Intellectual Property Agreement TRIPS ; . To render the Vietnamese law consistent with obligations of Articles 27 and 31 of the TRIPS Agreement which are incorporated in the U.S.-Vietnam BTA ; , Vietnam needs to include in its implementation package measures that specify that importation of a patented product will be legally equivalent to manufacturing of the product in Vietnam, and as a consequence, be sufficient to block the grant of a compulsory license based on non-use or inadequate use. In addition, the patent law should be amended to require "compulsory licensees" to pay a level of compensation commensurate with the patent's market value as provided in the TRIPS Agreement and the U.S.-Vietnam trade agreement. Infringement of Registered Trademarks Although the new Civil Code and associated implementing legislation provide a clear legal basis for protecting registered industrial property rights in Vietnam, infringement of registered trademarks is systematic and widespread, causing substantial financial losses to members of PhRMA. State-owned pharmaceutical companies under the jurisdiction of the Ministry of Health, and manufacturers and distributors from foreign countries figure prominently in infringement of the registered trademarks of PhRMA member companies.

Tonin transporter gene did not predict SRI response in OCD patients. When response to clomipramine or fluoxetine was considered alone, there was also no association with the 5-HTT polymorphism. This does not rule out the possibility that the polymorphism may be associated with SRI response in OCD patients, however. The retrospective nature of the study may not have provided accurate treatment response ratings for the patients. We also could not control for other medications that the patient could have been taking, such as buspirone or benzodiazepines. A prospective study, using Y-BOCS scores pre- and post-treatment and controlling for as many variables as possible, will help to provide a definitive answer as to the role of the 5-HTT gene in predicting SRI response. The importance of the trends in this study cannot be determined until there has been independent replication of the results, utilizing larger sample sizes. Although the 5-HTT promoter polymorphism may be associated with OCD in some patients, given the evidence for genetic heterogeneity, there are undoubtedly more genes that play a role in the disorder. There is a need to test more candidate genes, especially genes coding for components of other neurotransmitter systems and signal transduction pathways. Molecular genetic studies using these candidate genes may prove to be one of the most fruitful approaches currently available for unravelling the pathogenesis of obsessive compulsive disorder and
leflunomide.

Inadaequate eye contact and expressive language deficits were the primary target symptoms of this study, the treatment effect of greatest size was a 33% decrease of symptomatology. Interestingly, in a recent trial of clomipramine in autistic disorder, an improvement in the anger scale of the CPRS was the stronge, but unexpected, effect.27 There are very few specific clinical instruments that target the response of inadaequate eye contact and expressive language deficits to drug treatment in autistic persons. Both parents and teachers remarked that several of the items of the questionnaire were irrelevant for evaluating their autistic child. The failure of the clinician ratings to be sensitive to changes may have been influenced by several factors. Although clinician-rated effects at 1 month were equivalent to ratings at 2 months in a study of Quetiapine treatment of nonautistic children with attention deficit disorder and hyperactivity, 4 6 weeks may have been an inadequate duration to determine full Quetiapine effect in the current study. The environment in which clinician ratings were performed was not optimal to serve as a sensitive indicator of the modest effects of Quetiapine during the 6-week trial. The videotape equipment was a focus of curiosity and attention. Several of the subjects associated their visits with blood draws done after the videotaping. Some of the children verbalised their anxiety about the blood-drawing procedure during the taping sessions. In addition, the videotaping was done in a small office, which may have limited the subjects' movement. This highly structured and defined space was not the same as that in which the teachers and parents observed the children. It would have been preferable to make clinician ratings in each child's home and school setting. Because this may he impractical for most outpatient clinicians, ratings by teachers and parents arc invaluable in the assessment of children with autistic disorder during psychopharmacologic treatment. Possible "unblinding" of parents and teachers because of side effects is another limitation of the study. Videotapes were rated by the clinicians several months after each patient's treatment period ended, and clinicians were unable to make accurate assessments of which drug phase during review of the videotapes. Another methodologic issue of concern was possible carryover, which was minimised by a 1-week taper during the crossover period. In addition, a small number of subjects was involved in this study. Clinical implications: 8 of the 12 subjects were treated with Quetiapine in an open manner after the study. All of these 8 subjects continued to respond with a symptom and

Clomipramine users
As mentioned several times previously, the SSRIs are relatively benign in overdose. But there is one effect of these drugs that can happen with therapeutic use and after an overdose that is not so harmless: serotonin syndrome. The serotonin syndrome is a condition in which there is excess stimulation of the serotonergic receptors. No single serotonin receptor appears to be involved in the serotonin syndrome. It is most commonly seen after two drugs have been given that both affect serotonin reuptake, but it has rarely been reported to occur after one therapeutic doses and has been reported after overdose of single agents.14 The exact incidence of serotonin syndrome is not certain: post-marketing surveillance reported an incidence of 0.4 cases per 1000 patients months for patients taking nefazadone15 and one author estimates that 14 to 16 percent of patients that overdose on an SSRI will have serotonin syndrome.16 What causes serotonin syndrome? Any drug that 1 ; inhibits the breakdown of serotonin, 2 ; blocks the reuptake of serotonin, 3 ; acts as an agonist at serotonin receptors, and 4 ; causes release of serotonin. Nevertheless, the data for the efficacy of these agents in TTM is less convincing. Thus, the use of medication in the treatment of TTM deserves a more detailed examination than might be necessary if it were more similar to the treatment of OCD. Uncontrolled studies have suggested that SSRIs such as fluoxetine, fluvoxamine, and citalopram are useful in the treatment of TTM; in addition, controlled studies have shown efficacy for fluoxetine Piggott, L'Heueux, & Grady, 1992 ; and venlafaxine, a serotonin and noradrenaline reuptake inhibitor Ninan et al., 1998 ; . However, a number of placebo-controlled trials of SSRIs have been negative O'Sullivan, Christenson, & Stein, 1999 ; . Furthermore, studies examining the maintenance of gains in patients receiving SSRIs have found relapse of symptoms even in patients receiving maintenance pharmacotherapy Pollard et al., 1991; Benaroche, 1991 ; . Disappointing results with SSRI treatment for TTM have led to attempts to use other kinds of medication, either as monotherapy or as a way to augment serotonin reuptake inhibitors. Dopamine blockers are known to be useful in Tourette's disorder and are used as augmenting and rosiglitazone.

ABSTRACT. Purpose: To establish structural equation model SEM ; of subjected quality of life QOL ; in cancer patients taking into account qualification of pharmacists. Method: The SEM model was constructed from correlation matrix of the scores of answers of questions to both patients and pharmacists. Data were collected from 15 cancer patients who hospitalized and took opioid analgesics for pain control. The patients were asked 18 questions and pharmacists were asked seven questions. From the correlation matrix among scores of answers, a reasonable model was explored by SEM. Results: Health-related QOL HRQOL ; in cancer patients can be modeled by latent variables consist of contributions from physical, emotional and functional domains. The fitting between data and the model was acceptable by statistical goodness-of-fit GOF ; index. The modeled HRQOL by SEM was weakly correlated with subjected QOL in patients, indicating that subjected QOL in patients would be affected not only by above latent variables but other variables. The model taking into account qualification of pharmacists to improve subjected QOL in patients was also made by SEM. The model was reasonably explained and fitting between data and the model was acceptable from some statistical index. The final model suggests that pharmacist can raise subjected.

Steven Block, a biophysicist, argues that there is still "a lot of basic science work that needs to be done" and that scientists still "don't know how to design . complex macromolecules that work."83 Even Richard Smalley, a believer in the potential of nanotechnology to transform medicine, has doubts about nanorobots: "What they talk about doing with nanorobots is beyond even my own considerable imagination . Turn on the lights everybody; it ain't gonna happen like that."84 Critics argue that precise manipulation of atoms is extremely difficult. Even if individual nanorobots could be assembled, it may be impossible to produce billions of nanorobots necessary for commercial applications. Nevertheless, there is reason to believe that nanorobots will have some impact on medicine in the next 30 years. Montemagno maintains that within two years, he will be able to demonstrate the uses of his biological motor85, and Freitas avows that most of the work in designing and constructing nanorobots "should be complete within the next 20 to 30 years."86 Rapid advancements in the last few years demonstrate that nanotechnology "can go from fiction to reality in 10 years."87 Ultimately, only time will tell if nanorobots are more science fiction than reality. III. THE FDA A. Overview of the FDA's Regulatory Framework The FDA is the agency responsible for regulating the safety and effectiveness of most food products, human and animal drugs, therapeutic agents of biological origin, medical devices, radiation-emitting products, cosmetics, and animal feed.88 The agency, which operates under the Department of Health and Human Services, has a budget of $1.294 billion and 9, 100 employees.89 The FDA is organized into several centers that specialize in regulating particular types of products: the Center for Food Safety and Applied Nutrition CFSAN the Center for Drug Evaluation and Research CDER the Center for Biologics Evaluation and Research CBER and the Center for Devices and Radiological Health CDRH ; .
