There are now two health systems: a basic, universal health system and an "I'll pay for extra time" health system, for instance, clavulanate dose. Hospitals and long-term care facility pharmacies have been found to divert drug products to the retail market or to secondary wholesalers, drug diverters, or fraudulent drug re-packagers. These "closed-door" pharmacies have contracts with drug manufacturers stating that they are not to sell products on the open market and, in return, the closed-door pharmacies get favorable prices from drug manufacturers. However, some have taken these discounted drugs and sold the products to secondary wholesalers, drug peddlers, or drug diverters.34 Some of these operations have even diverted their "in-house use stock" to outpatient consumers. Employee theft from hospitals has also been a means to get expensive drugs to the "gray" market.35 Diversion schemes usually involve several people from a variety of operations and can involve many countries. Such schemes usually incorporate money-laundering operations where income for the diversion can easily be hidden. Examples include phony health care operations, store-front operations, or drug wholesalers that have been set up just to launder money from drug diversion schemes and drug counterfeiting activities. These are sophisticated operations with many having links to organized crime contacts and narcotic cartels. The draw of counterfeit drugs to prescription drug diversion operators is the huge payoff in profits. Drug diverters can generate large amounts of cash by diverting prescription drug products, especially stolen drug products, but larger sums of money can be obtained from counterfeit pharmaceuticals. As depicted in Figure 6, one approach is to mix or "cut" the diverted authentic drugs with counterfeit drug products. In examining how counterfeit drugs enter our drug distribution system Figure 6 ; , there is a common element found in many of the schemes: the secondary drug wholesaler or drug repackager!


The trial involves post-menopausal women with established coronary heart disease chd ; or with multiple risk factors for acute chd events and ampicillin. The mechanism of resistance was mainly the production of -lactamases, which was detected in all resistant isolates. ESBLs were produced in three isolates 1.5% ; , which had a resistance to cefotaxime and ceftriaxone, a susceptibility to amoxicillinclavulanate and ticarcillinclavulanate, a strong isolectric focusing band at isoelectric point 9.0, and a positive DDST. The prevalence of ESBLs varied among countries. Our isolates harbor ESBLs less frequently than in Turkey 27.7% ; , India 18% ; , Russia 15.8% ; , Cameroon 14.3% ; and Korea 9.3% ; 31, 3841 ; . In contrast, in agreement with our data, Canadian, European, Lebanese, Tunisian and U.S. investigations revealed that overall prevalences of ESBLs among E. coli isolates were 0.26%, 1.3%, 2%, and 3.3%, respectively 42-44, 24, 45 ; . ESBL producers observed in our study were found carrying the ESBL genes for CTX-Mtype -lactamases. As described previously, in contrast with the TEM- and SHVtype ESBLs, the CTX-M enzymes are much more active against cefotaxime and ceftriaxone than against ceftazidime. During the past decade CTX-Mtype ESBLs have emerged and increased worldwide in proportion to the other ESBL types, in single and epidemic clinical isolates 10, 11 ; . The detection of the blaCTX-M-3 in Algerian Salmonella enterica serotype Seftenberg isolates supports our results 46 ; . The three CTX-Mproducing isolates were from urine; E. coli CTX-M producers were described as the most common cause of urinary tract infections 47 ; . The ESBL phenotype may not have been transferable because these -lactamases may be carried on small nontransferable or large transfer-deficient plasmids. A small plasmid 7 kb ; , without genes required for conjugation, harboring blaCTX-M was described 48 ; , and ceftriaxone and cefotaxime resistance with ESBL production mediated by a small plasmid of 3.2 kb was reported in S. enterica serotype typhimurium 49 ; . However, the non-self-transmissible plasmids can be mobilized 50 ; . In addition, different elements, like insertion sequences and integrons, were also involved in the mobilization of blaCTX-M genes 51, 52.
