A Nurse Will Help Patients Manage Diabetes Once diagnosed by a physician, patients learn what needs to be done in order to control chronic disease. In the case of diabetes, that "what" involves a great deal of self-management. It also requires selfmotivation and behavioral change, and that's what presents the real challenge. How does a diabetic adapt? What steps must be taken? How do you change your bad habits? It may be self-management, but that doesn't mean doing it alone. The MPI Health Plan has enlisted the help of the Motion Picture & Television Fund MPTF ; to offer these Participants and eligible dependents the support of a personal health "coach." This coach is a specially-trained registered nurse who will work closely with them over the telephone to follow doctor's orders for their disease in order to achieve their individual goals. The RN health coach will also support communication with the Participant's physician. For individuals who are diabetic, this support can range from practical approaches to regulating blood sugar and blood pressure to tips on motivating oneself to exercise daily. In the role of coach, the registered nurse is a motivator and instructor, a professional willing to listen and address questions and concerns, for example, bromocriptine withdrawal.

The drop-down menus that complete the name of a drug after the first few letters are typed in were seen as useful by the doctors. Junior doctors were certain that the system made them more efficient, and cutting down on time waiting for paperwork to make its way to them from other locations. It was felt that this functionality would make prescribing more accurate, as it could be done in the light of up-to-date and accurate pathology information. The nurses pointed out that the clarity of written communications would be improved through the use of electronic reporting. Using the electronic system would avoid the common Another frequently mentioned advantage, and problem of legibility of prescriptions and

Phetamine was tested in a small, randomized trial in aphasia, which was not evaluated in the Cochrane review.241 The drug was beneficial for aphasic patients, but the beneficial effects did not appear to be sustained at 6 months. In retrospective analyses of data collected during stroke clinical trials, 172, 174, 175 and in animal studies of recovery from brain injury, 173 CNS depressants such as neuroleptics, barbiturates, benzodiazepines, and anticonvulsants have been associated with poorer outcomes. In the human studies, it is difficult to separate cause and effect, because the conditions treated by these medications, when occurring after stroke, may themselves be associated with more severe brain injury and worse outcome. In the absence of additional data, clinicians should limit the use of these medications in patients recovering from stroke as much as is practical. Routine use of these medications for minor indications eg, use of benzodiazepines for mild insomnia during inpatient rehabilitation ; is discouraged. Centrally acting 2-adrenergic receptor agonists such as clonidine and others ; and 1-receptor antagonists such as prazosin and others ; have been associated with poorer outcomes in at least 1 retrospective analysis. Model studies found poorer recovery in animals treated with clonidine and prazosin.173 Data support the beneficial effects of other classes of antihypertensives angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, and diuretics ; for secondary stroke prevention, and these drugs are generally preferred as first-line agents for hypertension control in patients after stroke.250 Consider bromocriptine or dextroamphetamine in selected aphasic patients. There are insufficient data on optimal dosing, and safety precludes the routine use of these medications for aphasia. EVIDENCE. See Table 37 and levodopa.
